Irritable bowel syndrome -A case report

ABSTRACT
Irritable bowel syndrome is as such functional disorder but it disturbs the person not only physically but mentally also. Due to it is disturbs the quality of life of person. Homoeopathy has greater emphasis on irritable bowel syndrome. Quality of life is improved by homoeopathic medicine with regulation of gastrointestinal motility with improvement on cognitive function of human being.

INTRODUCTION:A functional disorder is a medical condition that impairs the normal function, but -without major organic cause such as irritation or inflammation and where the organ or part of the body looks completely normal under medical examination. The accumulation of abnormalities that limit body functions is a major risk factor for patients with irritable bowel syndrome (IBS),defined as a gastro intestinal disorder with abdominal pain or discomfort that is associated with a change in bowel habit. Often, this disorder is accompanied by a concomitant decline in cognitive or motor performance.(1)

HOMEOPATHIC TREATMENT:

ASAFOETIDA(6)
Flatulence and regurgitation of liquid , Hysterical flatulence , Sensation of emptiness and weakness, with distention and beating in stomach and abdomen , Gurgling and rolling of wind, which escapes afterwards with loud and difficult eructation , Distended, griping, with hunger , Obstinate constipation. Diarrhoea, extremely offensive, with meteorism, and regurgitation of food.

NUX VOMICA(6)
Weight and pain in stomach; worse, eating, sometime after. Flatulence and pyrosis. Nausea and vomiting, with much retching. Epigastrium bloated, with pressure s of a stone, several hours after eating. Loves fats and tolerates them well. (Puls. opposite.) Wants to vomit, but cannot. Flatulent distension, with spasmodic colic. Weakness of abdominal ring region. Constipation, with frequent ineffectual urging, incomplete and unsatisfactory; feeling as if part remained unexpelled. Irregular,peristaltic action; hence frequent ineffectual desire, or passing but small quantities at each attempt. Alternate constipation and diarrhoea-after abuse of purgatives. Constant uneasiness in rectum.

ARGENTUM NITRICUM(6)
Belching accompanies most gastric ailments. Flatulence; painful swelling of pit. Ulcerative pain in left side under ribs. Watery, noisy, flatulent; green, like chopped spinach, with shreddy mucus and enormous distention of abdomen; very offensive. Diarrhoea immediately after eating or drinking.

COLOCYNTH(6)
Feeling in stomach as if something would not yield; drawing pain. Agonizing cutting pain in abdomen causing patient to end over double, and pressing on the abdomen. Jelly-like stools. Worse ; from anger and indignation. Better ; doubling up, hard pressure, warmth, lying with head bent forward.

complaints of unsatisafactory stool with inaffectual urging since 6 month. alter bowel habbit with constipation and diarrhea since 6 months. gurgling sound form abdomen since 5 months. all complaints agg from stress regarding bussiness.

heaviness of abdomen after eating any kind of food. eructation with distention feeling at stomach. nausea sometimes, no vomiting. no episode of gastric headache.

LIFE HISTORY: patient belongs to middle class family. born and brought up in surat. patient was average in study. more interest in father business and wants to join father’s business of diamond market. patient has one younger brother. they live in nuclear family. he completed his study upto 12th std and join his father business and since till date he is in diamond business. before six months patient having loss on diamond business and he has anger regarding that business loss. he has constant fear regarding money and business. all that complain started after business loss.

PHYSICAL GENERAL:

  • APPETITE: reduced since 6 months
  • THIRST: 2- 3 litre per day
  • DESIRE: spicy and oily food
  • AVERSION: nothing specific
  • STOOL: unsatifactory with alter bowel habbit
  • URINE: no abnormlaity
  • PERSPIRATION: scanty, only while more exertion
  • DREAM: nothing specific
  • SLEEP: disturb due to complaits and stress regarding bussiness loss

PHYSICAL EXAMINATION:

  • BLOOD PREESUURE: 134/84 mm/hg
  • PULSE: 87/min
  • RESPIRATION RATE: 18/min
  • CVS: hs1 hs2 heard
  • CNS: conscious and oriented
  • RESPIRATORY: B/L normal breathing pattern with no abnormality
  • GIT: gaseous distention and whole abdomen while palpation, on ausculation normal peristaltic movement, no specific finding on percussion, on inspection there are no sign of scar or suture marks or swelling. no sign of clubbing, cynosis, icterus.

PAST HISTORY:

  • Recurrent gastritis due to food habit
  • Taking allopathic medication for same complaints 3 months before.

FAMILY HISTORY:

FATHER: hypertension / alive / on medication

MOTHER: total knee replacement 5 year before

YOUNGER BROTHER: no abnormality.

ANALYSIS:

PRESCRIPTION

NUX VOMICA 200 BD 4 PILLS FOR 2 DAYS

S.L TDS FOR 15 DAYS

Then after Pt had a relief and in between two times Nux –Vomica was repaeated. But most important with physical symptoms quality of life was improved which was not possible with other mode of treatment which patient was taking since long.;

CONCLUSION(5)
The symptoms of IBS can be similar to those of other pathological conditions; it is necessary to exclude them by means of diagnostic examinations. By homoeopathic medicine and with proper diet and regimen we can improve quality of life in cases of IBS. Furthermore, a careful evaluation of the nutritional state is recommended with the aim of identifying, if present, the eventual lack of macro/micronutrients. Homoeopathic medicine improve psychological effect with cognitive function and gives better immune system.
